So can you clarify what is your current server version of CGPro, MAPI server version and client version that is now working with Outlook 2013?

Upgrading MAPI server has fixed the problem.

On a side note, I had uninstalled CGPMAPI on the client and was able to connect with Outlook (in safe mode) using an Exchange ActiveSync account. I have not reinstalled MAPI yet.

What are the benefits/problems with using ActiveSync instead of MAPI?
