I have only been using SIMS for a couple of weeks so I may be
misunderstanding something, but I thought once a connection failed to verify
it would be refused (?)


I believe that with "verify return paths" SIMS only verifies that the return path actually exists. In this case the mail was from qey23@eortc.be a MX look up on eortc.be yields:
